Here at Lake Josephine Family Dental, patients without insurance can sign up for our in-office savings plan. In exchange for a fee, you’ll enjoy a reduced price for cleanings, examinations, and other services.
Plus, since it’s not a typical insurance plan, there are no deductibles or annual maximums to worry about.

The in-office savings plan is an annual, renewable dental savings plan for families and individuals without other insurance plans.

• NO waiting periods to begin treatment.
• Benefits and savings begin immediately upon registration.
Prices & Fees:
The initial cost is $395 for one individual basic preventative plan or $550 for the periodontal plan. The discount for an additional family member in the same household is only $350. A missed appointment fee is $65.
Please notify our office at least 2 business days in advance if you must change your reserved appointment.

Does Not Apply With The Plan:
• Any previous treatment that has been completed. • Any procedures for full mouth rehabilitation requiring appliances for restorations. • Conditions or services under workers compensation or employee liability laws. • Any services provided for free by the county, government, municipality, or other agency. • Any procedures referred to another dental office. • Patients cannot use both dental insurance benefits and the in-house savings plan--this plan is for the uninsured patients only.
• There are NO refunds on your in-house plan.
• Co-pays on procedures not covered by plan due same day of service.
